Certain preventive care services are provided as specified by the Patient Protection and Affordable Care Act (ACA), with no cost-sharing to you. If you are participating in and meeting the requirements of a disease management program for diabetes, asthma, or CAD, you may not have to pay any co-pay or co-insurance payments for certain medications used to treat that condition. Click on your plan below to learn more.
Family Support Program (FSP) is a telephonic case management program to support the caregiver of a high needs’ pediatric member. Caregivers of children between the ages of 0 – 18 with complex behavioral health conditions will receive a dedicated 1:1 FSP Care Advocate who will first assess the needs of the family, and then provide tailored navigation, education, guidance, referrals and support to address the specific family needs.

The Diabetes Disease Management Program connects you with experts who can provide information to help you manage your diabetes, manage your medications, and improve your diet and exercise.
The program provides members with personalized, 1-on-1 support designed to address their needs throughout the care journey. Our case managers engage with SHBP members a minimum of every 30 days and every 90 days if their Diabetes is well controlled. Additionally, provider outreach is made to obtain clinical updates including notes, labs and other clinical indicators.
Our enhanced digital capabilities provide the right balance of education, tools and motivation as well as increased communication to help us more closely manage care for SHBP members. SHBP members can exchange messages with their nurse, access curated educational content and their nurse can also communicate on changes in health data (provided by employee) like blood pressure, weight and glucose.

An experienced bariatric nurse will work with you to help you make informed decisions and find a Bariatric Center of Excellence for weight-loss surgery with better outcomes and fewer complications.

The Centers of Excellence (COE) program providers and medical centers are independent contractors who render care and treatment to health plan members. The COE program does not provide direct health care services or practice medicine, and the COE providers and medical centers are solely responsible for medical judgments and related treatments. The COE program is not liable for any act or omission, including negligence, committed by any independent contracted health care professional or medical center.
Disease Management programs and services may vary on a location-by-location basis and are subject to change with written notice. UnitedHealthcare does not guarantee availability of programs in all service areas and provider participation may vary. Certain items may be excluded from coverage and other requirements or restrictions may apply. If you select a new provider or are assigned to a provider who does not participate in the Disease Management program, your participation in the program will be terminated. Self‑Funded or Self‑Insured Plans (ASO) covered persons may have an additional premium cost. Please check with your employer.
Well-being incentive credits you and your covered spouse redeem can be used for eligible medical and pharmacy expenses. You and your covered spouse can earn up to 720 credits each which includes the 240 UnitedHealthcare incentive credit match.

If you sign up for the Choice HMO, complete the wellness activities, and redeem your points through the Sharecare Redemption Center, your redeemed credits will be placed into your UnitedHealthcare HIA.
We will automatically use the money in your HIA to help pay for eligible medical and pharmacy expenses. This lowers the amount you have to pay.
Using the Health Incentive Account (HIA) with the HDHP with an HSA
If you sign up for the HDHP, complete the wellness activities, and redeem your points through the Sharecare Redemption Center, your redeemed credits will be placed into your UnitedHealthcare HIA.
Your points will be earned as activities are completed. To begin earning your points, complete the following activities between January 1, 2022 and November 30, 2022.
NOTE: Points cannot be awarded until completion of the RealAge Test. Biometrics, Well-being Coaching, Online Pathways and RealAge Program taken before completion of the RealAge Test can only be applied to points upon RealAge Test completion.

When do I receive my credits?
Choose from two options for redeeming points.
Regardless of how you redeem your points through Sharecare, UnitedHealthcare will match up to 240 additional credits* for you and your covered spouse to be used in your HIA to help offset eligible medical and pharmacy expenses. These additional credits awarded by UnitedHealthcare cannot be used toward the redemption of the $150 Prepaid Visa® Card.
* The UnitedHealthcare incentive credit match will be available after you redeem your well-being credits through the Sharecare Redemption Center.
What can I use my credits for?
If you sign up for the Choice HMO: You can use your well-being incentive credits to help pay your co-payments, your deductible, and other out-of-pocket expenses for claims submitted electronically through your healthcare provider. Credits will be available in your HIA 30 days after redemption.
Complete the wellness activities and redeem your points through the Sharecare Redemption Center, your redeemed credits will be placed into your UnitedHealthcare HIA. IMPORTANT: Before the money in the HIA can be used, you will need to pay for covered services until the following amounts have been paid toward your deductible.
After you pay the above amount, we will automatically use the money in your HIA to help pay for eligible medical and pharmacy expenses. This lowers the amount you will pay out of pocket.
Important: For members enrolled in the HDHP, well-being HIA credits cannot be used until the $1,400 (employee) or $2,800 (employee + spouse, employee + child(ren), or employee + family) thresholds have been met.
Additionally, you can continue to save money in an HSA for services that the HIA may not be reimburse. Remember, your HSA can be used to save for expenses you’ll have in the future.
Rollover your well-being incentive credits to a UnitedHealthcare plan.
Regardless of which UnitedHealthcare plan you choose, your unused well-being credits will follow you. Whatever you earn by participating in the Be Well SHBP well-being program administered by Sharecare you get to keep. Rollover credits from a prior year cannot be used to redeem Prepaid Visa® Card.
